1. Acceptance of Terms

By accessing or using 1836 Wellness services, you agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use our services.

3. Services Described

1836 Wellness facilitates the following services through its affiliated healthcare providers:

• Precision Diagnostic: A clinician-ordered executive panel drawn at the 100-marker entry set covering 12 health domains + 45-minute NP consultation + 7-Domain Composite biological age calculation + mobile phlebotomy, ordered as the first step of care
• Treatment Programs ($3,500–$7,500): Physician-supervised metabolic optimization protocols of varying duration and scope, including the Executive Reset, Metabolic Plus, and Total Optimization programs
• Maintenance Programs ($499–$999/month): Ongoing monitoring, quarterly blood panels, and protocol adjustments for patients who have completed a treatment program

All treatment decisions are at the sole discretion of the treating healthcare provider. Healthcare providers reserve the right to decline service to anyone they determine is not an appropriate candidate.

4. Eligibility

To use our services, you must:

• Be between 18 and 65 years of age
• Be a current resident of the State of Texas
• Meet the clinical screening criteria determined during the eligibility assessment
• Provide accurate and complete health information
• Have a valid form of payment

5. Payment Terms

Reserving your place with the practice requires a $699 charter membership deposit at the time of booking. Payment is processed securely through CardPointe by Fiserv. Treatment program pricing and payment terms are discussed during your results consultation. Your care team will recommend the appropriate program based on your blood work, and enrollment terms are provided at that time.

6. Medical Disclaimer

1836 Wellness services are not a substitute for emergency medical care. If you are experiencing a medical emergency, call 911 immediately. Our services are designed for metabolic optimization and preventive health. They do not replace your primary care physician or specialist care. Results vary by individual and are not guaranteed.

7. Patient Portal

Patient records, intake forms, and secure communications are managed through Cerbo, a HIPAA-compliant patient portal and EHR system. By using our services, you agree to create and maintain a patient portal account for the duration of your care.

8. Prescription Medications

• All medications require a valid prescription from a licensed provider after clinical evaluation
• Medications are for your personal use only and may not be shared or transferred
• Medications must be used exactly as prescribed and stored as directed
• Medications are dispensed by licensed pharmacies

9. Limitation of Liability

To the maximum extent permitted by law, 1836 Wellness LLC, its officers, employees, and affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from or related to your use of our services. Our total liability for any claim shall not exceed the amount you paid for the specific service giving rise to the claim.

10. Modifications

We reserve the right to modify these Terms of Service at any time. Changes will be posted on this page with an updated "Last Updated" date. Continued use of our services after modifications constitutes acceptance of the updated terms.

11. Governing Law

These Terms are governed by the laws of the State of Texas without regard to conflict of law principles.
