The amount you actually paid for your assessment ($499 founders, $599 standard, or $995 Comprehensive) is credited once toward Metabolic Reset or Total Optimization if you enroll within 60 days of your results review. The $299 Current-Care Records Review is not credited.
02 / Choose your care
One starting point. Different ways forward.
Hormone therapy can stand alone. A metabolic program is a separate choice, not a prerequisite.
HORMONE THERAPY
Hormones, with more context.
Menopause and perimenopause care for women, or testosterone evaluation for men. Understand options, agree on appropriate treatment and review your response.
Testosterone therapy (TRT) for men: $299 per month. Women’s hormone therapy (estradiol + progesterone): $399 per month.
A licensed clinician evaluates you and decides on treatment. Medication is prescribed only when clinically appropriate. Monthly care has a 3-month minimum, then continues month to month.
Weeks 13 to 24 of Total Optimization are the Guided Transition Review: three 15-minute clinician check-ins at about weeks 16, 20 and 24, one repeat lab panel around week 22 reviewed at the week 24 visit, and a written continuation plan at week 24 (monthly Core Metabolic Care or a graduation plan).
Upgrading later
Moving from Metabolic Reset to Total Optimization: the balance is $7,500 minus the amounts you have already paid. It requires clinician review and a written remaining-care plan, and must be elected by your week-12 review. There is no self-serve upgrade.
Not an automatic addition
More medication, higher doses, hormone therapy or monthly care. Each requires clinician review and a written agreement.
Need more than one kind of care? Add-ons for either program cover 12 weeks each: TRT add-on $650, HRT add-on $995, oral ED add-on $400. With Total Optimization, one TRT add-on covers 12 of the 24 weeks.
Share your concerns, history and goals. Your clinician orders Labcorp blood work, and urine testing when indicated, explains the findings and provides written next steps.
You can stop after your assessment. No treatment commitment.
Your clinician sets retesting checkpoints. This illustrates the care process, not a predicted health outcome.
YOUR PROGRESS, MADE CLEAR
See your progress. Understand your next step.
Your goals set the direction. Blood work, relevant urine testing and your health history help your clinician shape your plan. Follow-up findings help you see what is changing, beyond the number on the scale.
Your starting point. Findings explained in plain language.
Your progress. Relevant markers, symptoms and everyday life.
Your next step. Priorities reviewed with your clinician.

WHAT CHANGED
A small rise in March (6.1%), followed by a June value of 5.4%, within the normal HbA1c category (below 5.7%). Fictional example, not an expected treatment result.
WHAT TO DISCUSS
How does this trend fit with other labs, your history and the goals you agreed with your clinician?

MEDICAL OVERSIGHT
Physician supervision
Physician-led oversight with a background in women’s health, hormone therapy, medical weight management and integrative wellness. Experience includes clinical protocol development, advanced-practice provider supervision and chart review.
Clinical evaluations, prescribing and treatment decisions remain with the medical team.
Medical care is provided by Franklin Street Medical PLLC, a separately physician-owned practice. 1836 Wellness LLC provides administrative support. Founder Stephen Myshak supports the patient experience and business operations; he does not diagnose or prescribe.
You should not have to make sense of it all alone.
The value is what happens around the lab result: the conversation, the context and a plan you can use.
Maybe you want more energy for family, more confidence in your health decisions, or support through a changing stage of life. We start with what matters to you, without promising a particular result.
Understand the whole conversation.
Review your symptoms, history and appropriate testing together. A lab number is one part of the picture.
Leave with something useful.
A dedicated clinician results review and written recommendations help you understand the next step.
Make informed changes over time.
If you choose ongoing care, planned reviews connect your experience with repeat testing when indicated.
Where & when
Texas care. A practical timeline.
Houston-based telehealth for eligible adults ages 18–64. Be physically in Texas for your clinical video visit. Home-draw availability is confirmed for your ZIP code before booking. Labcorp is our primary laboratory.
Direct-pay care. Medicare and Medicaid are not accepted or billed.
First, arrange the right testing.
Bring your medications, supplements and recent lab reports. Your clinician may add or remove tests based on your history.
Then, review complete findings.
Timing depends on the ordered tests, specimen transit and laboratory processing. Your review is arranged after all results are complete; confirm the expected schedule before starting.
Next, measure and refine.
Repeat appropriate testing at the checkpoints in your plan. Compare findings with how you feel. The time to notice changes varies; program length is not a promised outcome deadline.
Before you decide
Clear answers. Less guesswork.
Can I start with just an assessment?
Yes. The Focused Assessment is $599 with an at-home blood draw included. Your clinician reviews the findings with you and provides written next steps. You can decide about further care afterward.
Do I need Metabolic Reset before hormone therapy?
No. Hormone therapy is a separate route. Your clinician evaluates suitability; a metabolic program is not required.
How is my assessment payment credited?
The amount you actually paid for your assessment ($499 founders, $599 standard, or $995 Comprehensive) is credited once toward Metabolic Reset or Total Optimization if you enroll within 60 days of your results review. The $299 Current-Care Records Review is not credited. If you already paid for a diagnostic or charter deposit under our earlier terms, you keep a full credit toward a program, with no new expiry.
Does Total Optimization come after a 12-week Metabolic Reset?
Total Optimization is 24 weeks in total: weeks 1 to 12 follow the Metabolic Reset plan, then weeks 13 to 24 are the Guided Transition Review. Moving from Metabolic Reset to Total Optimization: the balance is $7,500 minus the amounts you have already paid. It requires clinician review and a written remaining-care plan, and must be elected by your week-12 review. There is no self-serve upgrade.
How does monthly care work?
Monthly care (TRT, women's hormone therapy and Core Metabolic Care) has a 3-month minimum, then continues month to month. After the minimum, cancel with 30 days' written notice. A program never rolls into monthly care automatically, and you give written consent at checkout.
How do I reach someone between appointments?
Your care plan explains the secure contact method and planned reviews. Administrative staff coordinate logistics; clinicians make clinical decisions. Ask about a treatment concern before your next routine review. Telehealth messaging is not emergency care.
Can you promise a medication or a result?
No. A licensed clinician evaluates you and decides on treatment. Medication is prescribed only when clinically appropriate. Your goals guide care; no program promises symptom relief, weight loss or age reversal.
