    The Precision Diagnostic

    It's our flagship diagnostic service: a blood analysis that goes far beyond a standard physical. At the entry tier we test 100 markers, 81 blood markers plus 19 urinalysis markers, covering 12 health domains. Your results are distilled into 7 composite domain scores that produce your 1836 Functional Bio Age: a single biological age number. Urinalysis stays at 19 markers on every tier, and higher care tiers add blood markers to reach 112, 123, or the complete 125.

    A standard annual physical checks 10–15 basic markers: glucose, basic lipid panel, CBC. Ours checks 100 at the entry tier. We test advanced markers like ApoB (a better predictor of cardiovascular risk than LDL cholesterol), insulin alongside glucose (to catch insulin resistance years before diabetes), inflammatory markers like hs-CRP and homocysteine, and a full hormonal panel. Most importantly, we score everything using optimized clinical reference ranges. Not the broad "normal" ranges that often miss early dysfunction.

    Everything: mobile phlebotomy at your home, office, or hotel; the Executive Panel drawn at the 100-marker entry set covering 12 health domains; 7-domain composite biological age calculation; 7-domain composite health scoring; a personalized report; and a 45-minute results consultation with your care team. Your clinician orders it as the first step of your care and reviews every finding with you.

    Results are typically available within 5–7 business days after your blood draw. Once processed, your care team will contact you to schedule your 45-minute results consultation.

    Biological Age

    Biological age is an estimate of how old your body is functioning, independent of your chronological age. Someone who is 45 years old chronologically might have a biological age of 38 (aging well) or 55 (aging faster than expected). We calculate it from a validated, weighted 7-domain composite of blood biomarkers correlated with aging, mortality risk, and functional decline: a single score, not an average of separate algorithms.

    We score the 100 markers of the entry set organized across 12 health domains, then distill them into 7 composite domain scores: metabolic, cardiovascular, inflammatory, hormonal, organ function, blood health, and body composition. Each composite is scored 0–100 using optimized clinical reference ranges (not standard lab ranges). The weighted composite of all 7 domain scores produces your 1836 Functional Bio Age: a single number that reflects how your body is actually performing relative to your chronological age.

    Most biological age tests use a single algorithm that outputs one number with no context. Our 7-Domain Composite Score Engine analyzes the 100 markers of the entry set across 12 health domains, generates 7 composite domain scores (each 0–100), and produces a biological age grounded in how your body is actually functioning. The composite is fixed to that same 100-marker set on every tier, so your baseline and your re-test are always directly comparable. Instead of a single opaque number with no explanation, you get domain-level scores that show exactly what's driving your number, and what to do about it.

    Pricing & Payment

    Care begins with a clinician-ordered Precision Diagnostic, the first step of your evaluation with the practice. From there, your care team recommends the program that fits your results. Treatment programs start at $3,500 for our 12-week Executive Reset.

    We offer three physician-supervised treatment programs: Executive Reset ($3,500, 12 weeks), Metabolic Plus (starting at $5,500, 12 weeks), and Total Optimization (starting at $7,500, 16–20 weeks). Your care team will recommend the program that matches your blood work. You're never upsold to a tier you don't need.

    Yes. After completing a treatment package, monthly maintenance programs start at $499/month. These include quarterly blood panels to track progress, protocol adjustments based on new data, and ongoing physician oversight. Maintenance is optional. Your care team will discuss it during treatment if appropriate.

    Not at this time. 1836 Wellness is a concierge service. Patients may submit their blood work receipts to insurance or HSA/FSA for possible partial reimbursement; we provide documentation, but reimbursement is not guaranteed.

    The diagnostic fee is non-refundable once your blood draw has been completed, as lab processing begins immediately. If you reserve and need to reschedule before the draw, we're happy to accommodate. Treatment program refund terms are discussed during enrollment.

    Eligibility & Logistics

    Our protocols are designed for professionals and high performers ages 18–65 who want deeper insight into their metabolic health than a standard physical provides. All adults within our age range are welcome. You must be a current Texas resident.

    Medical services are regulated at the state level. The physicians and nurse practitioners of our affiliated, physician-owned medical practice are licensed in Texas, and 1836 Wellness's MSO structure operates under Texas Corporate Practice of Medicine guidelines. We plan to expand to additional states in the future.

    Not exactly. We're a precision metabolic optimization service. Many patients do experience changes in body composition as their metabolic health improves, but our focus is on the underlying biology: insulin resistance, hormonal balance, inflammation, cardiovascular risk. Weight changes are a downstream effect of metabolic optimization, not the primary goal.

    After booking, your care team schedules a licensed, background-checked phlebotomist to come to your home, office, or hotel at a time that works for you. The draw takes about 15 minutes. Samples are sent to our CLIA-certified lab partner for processing.

    Treatment & Clinical

    Your care is provided by the 1836 Wellness care team: licensed physicians and nurse practitioners of our affiliated, physician-owned medical practice. 1836 Wellness handles administrative and concierge services under a management services agreement; all clinical decisions are made by your licensed providers.

    We don't list specific medications publicly because your protocol is entirely driven by your blood work. During your results consultation, your care team will explain exactly what's recommended and why, based on your 125-Marker Executive Panel, not a cookie-cutter protocol. All medications require a valid prescription from a licensed provider after clinical evaluation.

    Yes. The Precision Diagnostic is clinician-ordered as the first step of your care, not a standalone lab purchase. Patients enroll with the practice, and your provider orders the diagnostic, interprets it with you, and builds your plan from what it reveals. It is the starting point of an ongoing clinical relationship, not a one-off test.
